Job Responsibilities:
- Monitor patient vitals, administer medications, and perform clinical procedures.
- Assist with patient assessments, wound care, and catheter maintenance.
- Collaborate on care plan implementation and report changes in condition.
- Educate patients and families on care procedures and post-discharge guidelines.
- Document care activities and maintain a safe, organized environment.
Qualifications:
- Valid Ohio LPN license or NLC.
- Strong clinical skills with meticulous attention to detail.

- Proficiency in infection control practices and patient safety protocols.
- Proficiency in electronic health records (EHR); familiarity with PointClickCare is preferred.
